Application

Thank you for your interest in becoming a student at Providence Cristo Rey High School. The application process can be simple!  Please call (317) 860-1000 to speak with our Admissions Department if you have questions about the process. 
  • A visit to Providence Cristo Rey High School
  • A completed application Application.pdf
  • A copy of the applicant's Birth Certificate
  • A copy of the applicant's Social Security card
  • A copy of the parent's most recent 1040 federal tax form and W-2 (s)
  • A signed copy of the Request for Records/Recommendation form Request for records.pdf
  • Immunization records
  • Completed PSAS (Private School Aid Service) form   English   Spanish
Once all of your application materials have been received, the Admissions Review Committee will make a decision on your acceptance within 3 weeks and you'll be notified in the mail.

The Admissions Department holds "Application Sessions" every Tuesday night from 5:00 pm - 7:00 pm
